Client Forms

These client forms have been digitized and placed on this site for your convenience. You will note they have been grouped into two distinct categories: the intake information itself that includes office directions, as well as the two HIPAA forms. You will be required to complete all paperwork prior to your initial session with me. This ensures that I can serve you better.

Please print the forms from your computer, complete and sign them where indicated, and bring them with you to your appointment. If you do not have access to a printer, please let me know and hard copy forms will be made available for you to complete at the office when you arrive. If that is the case, please arrive twenty minutes prior to your appointment time to give you the opportunity to complete all paperwork. Thank you.
